About Pair Team 

Pair Team is a rapidly growing healthcare technology startup that helps connect underserved communities to high-quality care. They partner with shelters, pantries, and other nonprofits to launch telemedicine clinics onsite to engage a hard-to-reach Medicaid and Duals population. Through these partners, they help vulnerable patients overcome barriers to care and get convenient access to the right physical, mental, and social support services. All these services are funded by Medicaid, allowing patients to receive comprehensive, life-changing care for free.

Challenge

As Pair Team grew from a small startup to a rapidly expanding business they faced increasing difficulty in visualizing and managing patient data across their network. They needed a scalable solution to capture medical histories for patients who often lacked consistent access to healthcare, as well as a means to track the effectiveness of their interventions and justify value to insurers and care providers.

Solution

Pair Team partnered with Good Data Studio to develop bespoke, scalable data solutions. Good Data Studio implemented three key components:

  1. Clinical History Integration: Integrated with Zus Health to pull patient medical histories from multiple external HIE and ADT sources into their data warehouse, Snowflake. 

  2. Scalable Clinical Reporting Suite: Designed a user-friendly dashboard to visualize patient journeys, track HEDIS measures, and implement reporting on dozens of care gaps. Pair Team is now able to quickly add new clinical outcomes and care gaps reporting while fully owning the infrastructure in house.

  3. Longitudinal Data Reporting: Implemented a data structure to account for changes in patient population over time, enabling accurate long-term performance assessment and cost reduction studies.

"Pair Team can now look up a full clinical history through our connectivity to health information exchanges in the areas we provide care," Micon explains.

“We can visualize this in our electronic health records service starting with the patient’s name and date of birth and often no other PII. The comprehensive medical histories, combined with the robust reporting infrastructure that Good Data Studio built, now provide us with detailed clinical outcomes reporting. This level of insight is typically very challenging to achieve for an early stage company working with a Medicaid patient population.”
